Why “near me” matters more for SPRAVATO than for most treatments
For most medications, how far you live from the pharmacy is a minor detail. SPRAVATO is different, and it is worth understanding why before you commit to it.
Esketamine cannot be taken home. Every dose is self-administered at the clinic under supervision, and every dose is followed by a minimum two-hour monitored observation period required by the FDA. You also cannot drive afterward. Combine that with a front-loaded dosing schedule and the travel adds up quickly:

That is roughly 12 visits in the first two months, each one needing a driver. A 45-minute drive each way turns a three-hour appointment into most of a day, eight times in the first month alone. Patients who live closer finish the induction phase far more reliably — and finishing induction is what determines whether the treatment gets a fair trial.
None of this is a reason to avoid SPRAVATO if it is the right treatment for you. It is a reason to plan the logistics honestly up front, which we will do with you on the phone before your first appointment.
You cannot get SPRAVATO just anywhere
Esketamine is a Schedule III controlled substance available only through a restricted federal program called the SPRAVATO REMS (Risk Evaluation and Mitigation Strategy). In practice that means:
- The treatment center has to be certified in the REMS program. Most psychiatry offices and primary care practices are not.
- The medication is shipped only to certified settings. A retail pharmacy cannot fill it for you to take home.
- You must be enrolled in the program as a patient before your first dose.
- Each dose is self-administered on site, in front of staff, and followed by at least two hours of monitoring with blood pressure checks.
- At-home esketamine is not permitted under any circumstance. Any service offering it is operating outside the program.
This is why the pool of genuine SPRAVATO providers in any given area is much smaller than the number of clinics advertising depression treatment, and why a “near me” search is a reasonable way to find one.
SPRAVATO and IV ketamine are not the same thing
Some clinics advertise “ketamine therapy” without distinguishing between the two. SPRAVATO is FDA-approved and usually billable to insurance. What your first visit looks like
- Benefits check and prior authorization. Before you come in, we verify your coverage and submit the prior authorization most plans require. We handle this in-house and will tell you what your expected cost will be.
- Medical screening. We review your medication history, blood pressure, and medical conditions to confirm SPRAVATO is appropriate and safe for you.
- REMS enrollment. You are enrolled in the SPRAVATO REMS program, a federal requirement before the first dose.
- Your first dose. You self-administer the nasal spray in a private room while staff observe, with blood pressure checked before dosing, around 40 minutes after, and again before discharge.
- Two-hour monitoring, then home with your driver. You stay until a provider confirms you are stable. No driving until the next day after a full night of sleep.
Insurance and cost
Because SPRAVATO carries FDA approval, it is generally billable to commercial insurance, Medicare, and Ohio Medicaid managed care plans — a meaningful practical difference from IV ketamine, which is off-label and usually self-pay. Plans typically want documentation that you have tried at least two antidepressants without adequate benefit.

Is SPRAVATO right for you?
SPRAVATO is FDA-approved for adults with treatment-resistant depression — depression that has not responded adequately to at least two antidepressants — either on its own or alongside a daily oral antidepressant. It is also approved, together with an oral antidepressant, for depressive symptoms in adults with major depressive disorder who have acute suicidal ideation or behavior. It is not a first-line treatment and is not approved for anyone under 18.
It is generally not appropriate for people with certain blood vessel conditions including aneurysmal vascular disease, a history of intracerebral hemorrhage, or uncontrolled high blood pressure. Sedation, dissociation, dizziness, nausea, and a temporary rise in blood pressure are common during the monitoring window and usually resolve before you leave. Plan for roughly two and a half to three hours on site. That covers check-in and a blood pressure reading, self-administering the nasal spray under supervision, and the FDA-required minimum two-hour monitoring period before a provider clears you to leave.
Can I drive myself home after SPRAVATO?
No. You cannot drive or operate machinery for the rest of the day after a dose, so you need to arrange a ride for every single visit. A family member, friend, caregiver, or rideshare all work. Most patients can drive again the next day after a full night of sleep.
Does insurance cover SPRAVATO in Ohio?
Usually yes, with prior authorization. Because SPRAVATO carries FDA approval, it is generally billable to commercial insurance, Medicare, and Ohio Medicaid managed care plans, unlike IV ketamine, which is off-label and typically self-pay.
